I've been very happy with Sound Settings from Amoeba. It provides a whole lot of control over your sound input/output with devices and applications.
Mac Automaters just did an episode with one of the founders and they laid heavy praise for the app's scriptability. Though I myself have not gone there. Yet.
It is a somewhat significant one-time purchase but I've no regrets. Suggest you check it out.
