Move folders from source to destination based on country name

I have a source folder of subfolders with each subfolder containing a country name (there may be multiple folders with the same country in the folder name). The destination folder contains a list of countries as well. I would like to move each folder from the source folder to the appropriate country in the destination folder. I'm not totally sure how to approach this, so I'd appreciate any help!

Example: (the actual folders have dozens of countries)

Might I recommend a program called Hazel. It is very good at searching for files and folders by "name contains...." It is designed to move, rename, tag and more, to files and folders. Subfolders are no problem for Hazel. Also works in the background and constantly monitors folders. You set up rules, and it does the rest.
