As an idea for future versions, check out http://www.sikuli.org/. It essentially lets you target mouse clicks and other actions based on the image on the button. For apps with dynamic toolbars (or dynamic web pages) this could be very handy....

Keyboard Maestro already has the ability to find an on-screen image.
Look in the ‘Image’ category in the KM-Action-Inspector.

Unfortunately I am running into a problem with that icon being visible in multiple windows. It seems like a separate question so I created its own thread - How to use found image on just the active window
